Based on the belief that treatment extends beyond the center — into aspects of everyday life.
NYPCC offers a comprehensive care coordination program, Gateway to Wellness, at all three of our centers in the Bronx, Bushwick, and East New York. Clients are assigned a “Care Coordinator” who oversees their healthcare and makes sure they have access to the services they need to maintain their health—including referrals for housing, benefits, education, medical appointments, employment and much more.
We coordinate:
- Housing referrals and assessment
- Education referrals
- Integration of medical and behavioral care
- Appointment scheduling
- Peer and family counseling/support
- Crisis intervention
To be eligible to participate in the Gateway to Wellness program, individuals must be Medicaid beneficiaries, age 21 or older, and meet one of the following conditions: be living with HIV/AIDS or have 2 or more chronic health conditions such as asthma, diabetes, heart disease, have a substance use disorder or have a serious mental illness.
