The transition from a residential setting to independent living can be difficult. We help make it successful.
For more than 40 years, NYPCC has partnered with the New York State Department of Health to provide mental health services at its three licensed adult homes—where individuals with serious and persistent mental illness live in a social setting. Here they are provided with the basic life sustaining services to enable them to take the step from inpatient treatment to independent living. NYPCC’s programs, in conjunction with case management and peer support services, aid in their rehabilitation and recovery, assisting them in re-entering the workforce and independent housing.
Through the use of evidence-based practices, NYPCC’s staff of mental health professionals enable these clients to gain insight into their mental illness, manage their symptoms, and acquire the skills necessary to re-enter the community and live and work in a more independent setting.
