Care Coordination

Support you can count on, when you need it.

NYPCC Care Coordinators ensure every client receives the wraparound support they need to thrive. Working side by side with therapists, Care Coordinators support clients throughout their treatment journey to promote stability, recovery, and independence.

NYPCC offers a comprehensive care coordination program, Gateway to Wellness, at all of our centers in the Bronx, Bushwick, East New York, and Queens. This program serves adults with mental health conditions by addressing both clinical and non-clinical needs that impact recovery, in a fully integrated and person-centered way.

Each client is assisted by a dedicated Care Coordinator who helps them navigate healthcare, connect to essential services, and build the supports needed to maintain health and remain independently in the community. Care Coordinators assist clients in accessing the full spectrum of services that promote wellness, engagement, and reduced hospitalization.

We Coordinate Support Including:

  • Housing referrals and assessments
  • Benefits navigation
  • Education and support for managing chronic conditions
  • Linkages to primary care, specialty care, and mental health and substance use outpatient programs
  • Collaboration with medical and behavioral health providers
  • Community connection and social reintegration
  • School and employment goal support
  • Transportation coordination and appointment reminders
  • Food assistance

NYPCC’s Care coordination is available to NYPCC clients, who receive Medicaid, are age 21 and older, are managing multiple chronic medical and/or behavioral health conditions, including though not limited to asthma, diabetes, heart disease, substance use disorder, serious mental illness (SMI), HIV/AIDS, or sickle cell anemia.

Care Coordinators collaborate closely with providers and therapists, joining portions of sessions when appropriate and maintaining regular communication, to ensure care remains aligned, responsive, and effective. We keep the process simple, supportive, and person centered. Together with the clinic and the community, we make whole person care possible.
